Hospital / Hospice / Nursing Home Notarization

Need a document notarized at a healthcare facility? We can help with that.

If you have a loved one in a managed-care facility such as a hospital, hospice, rehab, retirement community, or nursing home, we can meet you at their room and notarize their signature on a power of attorney, DNR, will, or any other important paperwork.

We make this process easy! Here’s how it works:

  1. Make sure the signer comprehends the underlying transaction on the the document and is willing to sign. If the signer does not comprehend and willingly want to proceed, we will not be able to notarize the document. Travel fee will still apply.

  2. Make sure the signer has a valid ID, such as a driver license, state ID, or passport. If the signer does not have an ID, a credible witness who does have an ID can be there to swear or affirm that the signer is who they claim to be. The credible witness must be someone who knows the signer but is not named as a beneficiary in the document.

  3. Look at the document to see if witnesses are required. If so, please note the the witness(es) cannot be anyone named in or benefiting directly from the document. Some medical facilities will allow staff members to witness documents, and some will not, so check with the facility in advance about that. The ideal witness would be a friend or neighbor. If you cannot find anyone, we may be able to wrangle some witnesses with advance notice.

  4. Make an appointment to have one of our Tucson mobile notaries come to the facility to notarize the document.

  5. We will identify the signer, watch them sign, and notarize their signature. If witnesses are present, we may or may not also notarize their signatures, depending on the wording on the notarial certificate provided.
